Applying for Phase I or Phase II of DSRP

The Disabled Student's Residence Program offers two (2) separate forms of services: Phase I and Phase II. Phase I students are new to independent living; they reside in the dorms and receive direct, and personal care from DSRP staff (the bulk of the DSRP website covers Phase I services). Phase II students live where they choose to in the community (often the dorms, co-ops, or apartments). Phase II students are more independent, requiring only intermittent support from DSRP staff. Phase II services include all Phase I services (attendant referral, workability IV program connection, service navigation, housing referrals), with the exception of no personal care from S.A.s. Phase II students need not have gone through Phase I—they only need to be DOR clients and current Cal students with a disability.

Please review the DSRP eligibility requirements before starting the application process. Fill out the applications and forms for the specific Phase program of DSRP you wish to apply for. If you need assistance with the forms, or the application process, please don't hesitate to contact DSRP staff.

Application Process

  1. You must sign up with the California Department of Rehabilitation (DOR) before you can apply to the Residence Program.
  2. Download and fill out the DSRP forms (please note the phase indicated next to the form):
